You're muddling the issue. It is always and everywhere the case that the extreme ranges of the curve, the morbidly obese and the underweight, die early.
But now that I think about it, since there are roughly twice as many Americans who are underweight than morbidly obese, why aren't we scaring skinny folks into gaining weight ? I mean, it's not clear that losing weight is helping the morbidly obese live longer, but maybe it won't kill the underweight to eat a sandwich or two.
